PAYMENTS

Out-of-pocket fees such as copayments, coinsurance and deductibles are due at the time of service. Cash or credit cards are accepted, including health savings and flexible spending accounts.

CANCELLATION POLICY

If for any reason you do not provide 24 hours notice or do not show up for an appointment, you will be charged a late fee of $95.
